If you’re enrolling in a Chronic Care Management (CCM) program, you may not know what to expect once you sign up. And, this may mean you have some additional questions to see if the program is a good fit for you. With its numerous benefits in helping you maintain and improve your health, it’s still important that you find out everything you can about CCM before enrolling. What is chronic care management?

Chronic Care Management (CCM) is a program designed for people who participate in Medicare and live with two or more chronic, or health, conditions. These conditions can include asthma, arthritis, high blood pressure, heart disease or diabetes, among many others.

Since it can be overwhelming to keep up with several health conditions while also working with multiple healthcare providers, CCM can help save you time and energy. By providing you a dedicated healthcare team, you’ll receive consistent, personalized care that is shared with your healthcare provider. This way, you’ll spend less time managing your health and more time doing what you love.

What makes enrolling in the Wellbox CCM program different?

What will we talk about during the calls?

Calls are made by our team of registered nurses, the Chronic Care Coordinators (CCC), and they will discuss and develop a care plan with you. This plan can include lifestyle recommendations such as ways to maintain your blood glucose levels or your blood pressure, overall health monitoring and assistance with scheduling specialist appointments.

The calls are personalized to you and your needs, and they have shown positive results. For example, our nurses have helped patients find valuable resources and support on the monthly calls. They assist patients in saving on prescription medication costs, deliver more frequent touchpoints that help prevent people from being admitted or readmitted to the hospital, and have provided needed and valuable support to people.

Through a partnership with your healthcare provider, your nurse has full access to your medical records and they will update your health record after every call so your doctor has more visibility into your health. This can lead to better, more informed office visits and care.

How much will CCM cost?

After deductibles are met, most secondary insurances cover the remainder of the costs for the program. If your secondary insurance does not cover CCM, it costs an average of $8-10 per month.  Far less than an emergency room visit, hospitalization, or even a gym membership and far more effective at keeping you healthy!

 

If I enroll in two CCM programs, does Medicare pay for both?

No, Medicare only pays for one program at a time. If you are enrolled in a second program, we suggest contacting your Medicare Administrative Contractor (MAC) to verify the details of your plan and payments, but Medicare will likely pay for whichever provider bills first.

 

What do I have to do during the program?

All that is required for the CCM program is your willing participation to answer calls from your nurse and follow their recommendations and care plan for you. Our goal is to help you manage your health and conditions, and by working with our nurses, you can begin reaching your health goals.

 

If my provider is partnered with Wellbox, am I automatically enrolled in CCM?

You are not automatically enrolled in CCM, but you can ask your provider about participating in our program. If you have received a letter/mail/phone call from Wellbox, then your healthcare provider has identified you as eligible for CCM services. When we reach out to you, one of our enrollers can guide you through the process and help you get started on your care management. The program is completely voluntary and you can opt-out at any time.

 

What does my doctor think of the program?
