CCM is a program built for individuals that participate in Medicare living with multiple long-term, or chronic, health conditions, such as asthma, arthritis, high blood pressure, heart disease or diabetes. If you qualify for the program, you can receive 20 minutes of remote care management on a monthly basis from a qualified healthcare provider. CCM services include:
• Personalized attention by your Chronic Care Coordinator each month
• A direct line of communication between you and your provider via the Chronic Care Coordinator
• Consistent help and encouragement with any and all health goals you may have
Why did I receive a letter/email/phone call from Wellbox?
Based on your health profile, your healthcare provider has identified you as eligible for care management services. These services are provided on a monthly basis via phone and or mail and are in addition to your regular office visits. Your privacy in all these exchanges is secure, and in compliance with the Health Insurance Portability and Accountability Act (HIPAA).
Why should I participate?
The CCM program provided by Wellbox has been shown to have a positive impact on health outcomes for its participants — including fewer emergency visits and hospitalizations. The goal of the program is to anticipate and address health concerns or complications earlier when they are easier to treat, or even prevent.
Who will I be talking with? What will we talk about?
A team of registered nurses, acting as an extension of your existing practice, will provide care management services. They will be familiar with your medical history and will update your health record after every interaction so your primary physician can review the content of your interaction, and direct your care as he or she sees fit.
Discussions and patient education topics will include your care plan, prescriptions, other preventative services available to you, lifestyle recommendations, and assistance with scheduling specialist appointments.
How much does Wellbox CCM cost?
After you’ve met your deductible, Medicare covers 80% of the associated costs of CCM. Most secondary insurances cover the remainder of the costs however if your secondary does not cover the cost or you do not have secondary insurance you may be responsible for a small monthly co-pay.
Who are Dr. Weigel and Dr. Gladd?
Wellbox employs experienced doctors to supervise and advise on the Wellbox CCM program. Dr. Peter Weigel is the supervising physician for our Medicare CCM program and Dr. Jeffrey Gladd is the supervising physician for our Employee CCM program.
Dr. Peter Weigel is board-certified in Internal Medicine. He attended Johns Hopkins University and the University of Medicine and Dentistry of New Jersey. He practices at Medical Associates of Westfield and is currently on staff at Overlook Hospital and Robert Wood Johnson University Hospital at Rahway. He is also the medical director at Genesis-Westfield Center. He is associated with the American College of Physicians, New Jersey Medical Society, and Union County Medical Society.
Dr. Jeffrey Gladd graduated from Indiana University School of Medicine in 2001 and went on to train in family medicine at Ball Memorial Hospital in Muncie, Indiana. After completing his residency, Gladd started a family practice that encompassed full spectrum care in Columbia City, Indiana. Gladd went on to complete the University of Arizona Integrative Medicine fellowship program. While in the fellowship, Gladd served as the medical director of the Parkview Center for Integrative Medicine in Fort Wayne, Indiana, and in 2010 opened GladdMD Integrative Medicine where he blends health-promoting care with high access technology.
