Since 2015, the prevalence of Chronic Care Management in the U.S. has only continued to grow. The ultimate goal of CCM is to reduce avoidable hospitalizations, improve health outcomes, and enhance the patient experience between in-person visits. 

CCM has quickly shifted from a helpful service to a healthcare imperative, and a decade later, we’re approaching another inflection point. By 2030, nearly 70 million Americans will be aged 65 or older, and a majority will face at least one chronic illness. In fact, 90% of the nation’s $4+ trillion annual healthcare spending already goes towards people with chronic or mental health conditions. That burden is only growing, along with the pressure on providers to manage it efficiently and effectively.

Policy Backing and Payment Reform 

The foundation of today’s CCM growth was built on policy. Medicare first introduced dedicated CCM reimbursement in 2015, and support has only grown stronger. CMS’s 2024 rule changes expanded billing eligibility for FQHCs and RHCs, while proposed Medicare copay eliminations aim to remove patient financial barriers altogether. 

Add to that the increasing number of private payers and Medicaid programs embracing CCM codes, and the signal is unmistakable: policymakers and payers alike recognize the long-term value of investing in proactive care.
