Wellbox was engaged by UGL in February of 2018 to deliver Chronic Care Management (CCM) services to their eligible patient population. In September 2019, the team at UGL conducted an analysis of clinical outcomes to determine the efficacy of the program. They looked at each of their key quality measures, comparing the Wellbox population against their total Medicare population and their total primary care patient panel.

Medicare patients with two or more chronic conditions were offered participation in a CCM program that paired them with a dedicated registered nurse. As part of the program, the nurse performed proactive outreach on a monthly basis that included developing and managing against a personalized care plan. Care plans and nurse outreach topics included but were not limited to setting goals and objectives, medication reconciliation, care coordination among various providers, risk assessments, and preventative care recommendations.

After partnering with Wellbox to offer its CCM solution, UGL saw a documented impact within its patient population, specifically its diabetic Medicare population. It has seen these outcomes:

Increased Patient Engagement

Wellbox patients exhibited 117% the rate of engagement in comprehensive diabetes care.

Decreased Hemoglobin Levels

12% of Wellbox Diabetes patients had an A1c level of less than 9 compared to 21% of diabetic patients in the full Medicare Population at Upper Great Lakes Family Health Center.

More Preventative Care

This same practice also saw an increased rate of preventative care screenings among Wellbox participants.

After partnering with Wellbox to offer its CCM solution, Upper Great Lakes reports positive outcomes within its diabetic Medicare population.
