November is Diabetes Awareness month. In addition to discussing your patient’s usual health plans, this month, all our Care Coordinators are taking the time to educate your patients on this life-threatening disease. As 1 in 11 Americans has diabetes, we want to ensure we provide the best care not only for patients with diabetes, but for at-risk patients as well.

With over 86 million Americans being at risk of diabetes, our priority is to ensure your patients know the risks and are aware of the various warning signs. We help patients pursue better eating habits and incorporate physical activity in their schedule to try and empower positive lifestyle changes.

In the time it took you to read this short article, someone in the U.S. was diagnosed with diabetes (1 in every 23 seconds). We know the risks, and preventing diabetes and arming your patients with the tools to manage their disease effectively is our priority.
