June is Dementia Awareness Month. Therefore, in addition to the things we usually discuss about your patient’s health, we are also talking to all of your patients about dementia.

Alzheimer’s disease and other forms of cognitive decline are the 6th leading cause of death in the United States. More research is coming out that shows ways to reduce the risk of developing dementia and how early intervention is key.

For that reason, we will be providing dementia screenings for all of your patients this month and discussing ways to lower your risk of dementia through healthy lifestyle choices. How we do this is by conducting a short screening that entails a number of questions for the patient to respond to. After the AD8 Dementia Screening, the Chronic Care Coordinator then documents the assessment in the patient’s EHR. The patient receives education reviewed on lifestyle modifications for reducing the risk of dementia. The Chronic Care Coordinator must attempt to schedule a follow-up appointment if a patient receives a high score. If no appointment is indicated and not scheduled, there must be a reason listed. All education points that are discussed with the patient are also documented within the patient’s chart.
